For PKI certificate renewals, an updated Annex 1 document needs to be completed and sent (along with a copy of your company registration document) in your request through the Jira Service Desk. Please change the word 'Template' to your company name when saving the updated document to your system (please don't save it here). Also please remember to pay particular attention to section 4.6 of the document - as the cell phone number you provide will be used to send the PIN code and the email address you provide will be used to send the link and enrolment guide. If there are no changes to the contacts or company/organisation details in the original signed Annex 1 document, please disregard these templates and use the original to attach to your request in the Jira Service Desk.
